I'm can't put into words how disapointed I am

Hello: My live in girlfriend of 3 years went out with one of her girl friends, got drunk at a local bar, and never came home. We have a distinct policy of communicating if one is late, or in this case which by the way is the first ever, just not coming home at all. I don't want to end the relationship, but I'm can't put into words how disapointed I am. How should I approach this issue?

I assume she's back home. I find it hard to believe that you haven't talked to her about it by now. This is simple. Sit her down and say, why didn't you call? We have an agreement that says if you're going to be late, call. She'll say something to the effect of: "Yes. I know. I'm sorry." in which case you can take her apology or not and forgive her for the first time offense.
You don't want to end the relationship? How the heck did you get to her not calling to ending the relationship? You can't possibly expect such perfection that one infraction will be enough for you to call the whole thing off. You can certainly express your disappointment, you were worried about her safety, but it doesn't reach the level of ending the relationship unless you have an extreme need to be in control or unless this may be the first time she's not come home, but not the first time she's been late without calling.
